Changing Your Legal Name
Students who marry, divorce or by court order change their name and wish to change the name on their permanent academic record must do so with the Registrar's Office. The student should submit a Change of Legal Name form to the Registrar's Office. The request must be accompanied by a social security card bearing the new name.

Personal travel is required of students for all clinical courses. Liability for driving to and from clinical sites falls on the owner of the vehicle. Automotive insurance is required. Student pregnancy or a change in health status is to be reported to the course coordinator. A physician's consent is required for students to continue clinical agency experiences. After surgery or other hospitalization, a physician's release to return to clinical is required.
